Alignment Healthcare, an integrated health care services company, announced today that it has entered into a partnership with St. Vincent’s HealthCare, a ministry of Ascension, the nation’s largest non-profit health system. Alignment will provide an advanced clinical model, designed for Medicare beneficiaries, that promises to lower cost and increase care quality for seniors.

The newly formed Medicare Advantage program is the first health system-based platform in the region. It will bring concierge level healthcare services to the citizens of Duval and Clay counties in Florida. Medicare consumers will receive access to personalized care through dedicated clinics focused on coordinated healthcare delivery with their primary care physicians.

Of the approximately 170,000 Medicare-eligible seniors living in Duval and Clay counties, 30 percent are currently enrolled in Medicare Advantage plans. Medicare enrollees will have options to select this program during the open enrollment period that begins in October of 2015. 

To care for the area seniors, a select network of St. Vincent’s HealthCare providers and hospitals will be available to members. Newly-constructed one-stop coordinated care centers will serve as the foundation of the provider network in Duval and Clay counties. Community physicians and other care providers will also be in the network to ensure seniors have access to a full continuum of care close to home.
